Перспективные направления исследований.




Низкое качество доказательности изложенных рекомендаций связано с ограниченностью данных связывающих диагностические стратегии с лечебными результатами. Этот феномен связан с редкостью заболевания и отсутствием унифицированности диагностических методов. В дополнение к этому, публикуемые данные больших территориальных референсных центров, основанные на диагностике большого числа создает впечатление о большей распространенности заболевания и простоте его диагностики. Эти проблемы выдвигают на первый план потребность в дальнейшем исследовании диагностических критериев гиперкортицизма. Предлагаемые направления работы могут улучшить результаты лечения больных гиперкортицизмом:

1. Объединение информации. Создание национальными и международными эндокринными организациями базы данных пациентов, проверенных на синдром Кушинга, учитывая предполагаемое объединение диагностической информации. Эта объединенная информация помогла бы определить дифференциально-диагностические признаки и обосновать точные стратегии тестирования.

2. Стандартизация исследований. Диагноз синдрома Кушинга критически зависит от качества лабораторной диагностики кортизола в биожидкостях RIA, ELISA, или LC-MS/MS. Клиницисты нуждаются в большей специфичности методов, точности публикуемых диагностических критериев (cut-off). Национальные референсные лаборатории должны использоваться в трудных случаях.

3. Улучшенные качества клинических данных и проспективные клинические исследования. Первичная диагностика гиперкортицизма должна отвечать конечной цели любой диагностики - оказывать положительное влияние на лечебные результаты. Необходимо изучение результатов лечения, проведенного по современным лечебным стратегиям. В частности, существуют противоречия о необходимости оперативного лечения синдрома субклинического гиперкортицизма, выявленного у больных с инциденталомой надпочечника. Необходимо выполнение рандомизированных клинических испытаний, для выработки оптимальной лечебной тактики.
